This set of documents (in French) presents two tools for assessing caregiver needs and the associated risks, each accompanied by a user guide. 

The Grille de dépistage auprès des personnes proches aidantes, a five page screening tool, systematically determines the urgency of intervention required for caregivers. Its user guide (5 pages) is intended to help professionals and practitioners integrate the tool into their practice.   

The second tool, AIDE-Proches (22 pages), systematically assesses caregiver needs using a conversation-based approach. It is accompanied by an 18-page user guide. 

The tools and their guides are adapted from the materials developed under a large-scale national research project funded by Health Canada, entitled “Development of Screening and Assessment Tools for Family Caregivers” (Guberman, Keefe, Fancey, Nahmiash & Barylak, 2001).
