This webinar (1h09, in French) aims to raise awareness among professionals of the added value of involving caregivers in hospital care.  

The first part highlights two projects carried out under Measure 48 of the Quebec government’s 2021–2026 action plan for caregivers: one focused on partnering with caregivers in the context of hospital liaison, and the other on hospital discharge planning. The second part of the webinar showcases promising practices from across the province. 

The webinar first aired on May 8, 2025 as part of the Communauté de pratique Proche aidance.
