This report (59 pages) provides a conceptual framework for understanding and mapping the range of community-based care and support services that should be available to meet the health and social support needs of people with disabilities and older adults. It also analyzes the support provided by caregivers, and concludes with policy recommendations to strengthen community-based care services and supports across the European Region. 

This report was developed based on an extensive review of the literature, public policy documents and best practices worldwide. A technical expert group comprising specialists from various fields was also consulted.
